Professional Development Approval Form: Use to ask your BLT for a professional development you wish to attend individually as well as with a group. Your completed form is then turned in to your BLT for them to consider. Make sure to be detailed in your information and costs of the request. If you have a pamphlet or written explanation of the PD make sure to attach this information. If traveling, or wish to stay overnight, make sure to include the costs of these with your request. If a sub is needed make sure to include this information in your request as well.
